How Do I Apply for WIC?

Request a WIC appointment online or you may call 602-506-9333 to speak directly with a staff member. Please bring all family members applying for WIC to the appointment, along with one item for each type of the following documentation:

  1. Identification: Bring ID for each person in the family applying for WIC.
    • Birth Certificate
    • Child shot/immunization record
    • DES approval letter for AHCCCS, SNAP (food stamps), Section 8 Housing, and/or TANF
    • Driver's license
    • Hospital crib card
    • Hospital record
    • Military ID
    • Passport
    • Paycheck stub
    • School ID card
  2. Address (where you live): Bring a document that has your name and current address on it.
    • DES approval letter for AHCCCS, SNAP (food stamps), Section 8 Housing, and/or TANF
    • Driver's license
    • Lease agreement
    • Paycheck stub
    • Rent receipt
    • Signed letter from the person you are living with that has the address written on it
    • Utility bill
  3. Income: Bring proof of your entire household income.
    • DES approval letter for AHCCCS, SNAP (food stamps), Section 8 Housing, and/or TANF
    • Paycheck stubs for everyone in the household who works
    • Signed and dated statement from your employer showing your income
    • Unemployment letter/notice
    • W2 or tax forms
  4. Shot/Immunization Record: Please bring if you have a child less than 2 years old.

NOTE: A DES approval letter for AHCCCS, SNAP (food stamps), Section 8 Housing, and/or TANF counts for all three types of documentation. However, if you are not enrolled in any of these programs, you may still qualify for WIC.
