- Signed letter from the person you are living with that has the address written on it
- Utility bill
- Income: Bring proof of your entire household income.

- Paycheck stubs for everyone in the household who works
- Signed and dated statement from your employer showing your income
- Unemployment letter/notice
- W2 or tax forms
- Shot/Immunization Record: Please bring if you have a child less than 2 years old.
NOTE: A DES approval letter for AHCCCS, SNAP (food stamps), Section 8 Housing, and/or TANF counts for all three types of documentation. However, if you are not enrolled in any of these programs, you may still qualify for WIC.
